Tele-ICU Study at Baptist Health South Florida Reveals Positive Patient Impact in PCU Setting

Observational Controlled Study with 19,530 Patients Finds that Patients in Step down Units can benefit from Tele-ICU Monitoring

MIAMI, FL – June 6, 2017 –While the benefits of tele-ICU monitoring have long been established in the critical care setting ― picking up small and subtle changes in a patient’s condition so that problems may be proactively treated ― little research had been done in the use of remote monitoring in Step down ICU beds.

A study by Baptist Health’s tele-ICU department reveals that patients in Stepdown ICU’s, can benefit from monitoring by tele-ICU. The observational case controlled study, conducted between December 2011 and August 2016, looked at well-established outcomes and quality measures from 19,530 inpatients at six of its hospitals.

The findings showed significantly decreased mortality and a shorter length of stay in tele-ICU patients in the step-down setting. “With higher numbers of critical care patients, Step down ICUs are seeing increasing patients with higher acuity levels,” said Louis Gidel, Ph.D., M.D., Baptist Health’s chief medical informatics and quality officer. “Tele-intervention is an effective tool to improve patient care.”

Study outcomes provide evidence of the effectiveness of tele-innovation and validate the impact on quality and cost in the progressive care setting, which support a rationale for extension of access to tele-ICU care services across broader hospital populations.

Further investigation is needed to examine influence of tele-ICU care service on severity adjusted predictions across varying practice settings.

The research project was led by Donna Armaignac, Ph.D., R.N., and the study abstract was published in Critical Care Medicine.

Remarkable Patient Story: 48 Year-Old Homicide Detective Survives Stroke

Baptist Health Neuroscience Center Recognizes May as Stroke Awareness Month

MIAMI, FL – May 15, 2017 – May is nationally recognized by the American Heart Association and the American Stroke Association as stroke awareness month. Stroke is a serious disease that ranks fifth in causes of death and disability when compared to heart disease, cancer, respiratory diseases and accidents.

Most strokes are preventable and if treated quickly may make the difference between life and death – or the difference between a full recovery and long-term disability such as paralysis on one side of the body, speech or vision problems or memory loss.

When a stroke happens, every second counts – as in the case of David Denmark.

David, a 48 year-old homicide detective in Miami-Dade, was at a restaurant one evening when he experienced sudden stroke symptoms. “All of a sudden I couldn’t move … I couldn’t open my eyes because of the vertigo…and then it went downhill fast, really fast,” he says.

He was transported to Baptist Hospital’s Comprehensive Stroke Center where he was met by an expert team led by Sergio Jaramillo, M.D. “When Mr. Denmark entered the Center, we obtained a CT of the brain, assessed him neurologically and came up with a score that tells us how severe the neurological deficit is,” Dr. Jaramillo said, a neurologist with Baptist Health Neuroscience Center. “If the patient presents within three hours, we can consider them a good candidate for TPA like Mr. Denmark.”

TPA is a clot buster, a medicine that breaks up the clot wherever it may be. According to Dr. Jaramillo, David responded remarkably well and within 24 hours all of his neurological symptoms had resolved and there was no evidence he would have any permanent effect on his quality of life. “Every day I wake up and I can move freely. I constantly think what it could have been like. I know how lucky I am,” David says.

Dr. Jaramillo goes on to say that “If he hadn’t come to our comprehensive stroke center, there likely would have been some kind of permanent neurological deficit and would have had a negative effect on his quality of life”. That’s why the Joint Commission, which accredits U.S. healthcare organizations, along with the American Heart Association and American Stroke Association, have maintained a unique alliance since 2003 to promote stroke care in hospitals across the nation.

More than two years ago, Baptist Hospital in conjunction with Baptist Health Neuroscience Center, joined an elite group of facilities in the United States to be designated by the commission as a Comprehensive Stroke Center. The Joint Commission’s highest level of stroke care, an Advanced Certification, is awarded to those hospitals that have specific abilities to receive and treat the most complex stroke cases.

To achieve its status as a Comprehensive Stroke Center, Baptist Hospital underwent a rigorous onsite review by Joint Commission experts. The hospital was shown to have the necessary requirements, including:
• Neurologists, neurosurgeons and interventional neuro-radiologists;
• Advanced imaging capabilities;
• 24/7 availability of a neuro-intensive care unit with dedicated beds for stroke patients;
• A clinical staff with the unique education and competencies to care for patients with the most complex cases of stroke.

This month, Baptist Hospital in conjunction with Baptist Health Neuroscience Center was awarded the American Heart Association/American Stroke Association’s top award: Get With The Guidelines®-Stroke Gold Plus Achievement Award with Target: StrokeSM Honor Roll Elite Plus. The award recognizes commitment to providing the most appropriate stroke treatment according to nationally recognized, research-based guidelines based on the latest scientific evidence.

Baptist Hospital Achieves Magnet® Recognition for the Fifth Time and Reinforces a Commitment to Nursing Excellence

Miami, FL — May 1, 2017– Baptist Hospital attained Magnet recognition for the fifth consecutive time, a testament to its continued dedication to high-quality nursing practice. The American Nurses Credentialing Center’s Magnet Recognition Program® distinguishes healthcare organizations that meet rigorous standards for nursing excellence. The designation is the nation’s highest form of recognition for nursing excellence and a benchmark for the quality of care patients receive.

Baptist Hospital is part of the less than 1 percent of hospitals in the world to receive a fifth designation. In addition, only 22 hospitals in Florida and less than 7 percent of the hospitals in the nation have achieved Magnet recognition. Baptist Hospital was the first in the state of Florida to receive the prestigious Magnet recognition in 1998.
“Magnet recognition is an incredible honor and reflects our commitment to delivering the highest quality of care to this community,” said Bo Boulenger, the hospital’s chief executive officer. “We’re a better organization today because of the Magnet recognition we first achieved 19 years ago.”

The Magnet Model provides a framework for nursing practice, research, and measurement of outcomes. Through this framework, ANCC evaluates applicants across a number of components and dimensions to gauge an organization’s nursing excellence. The foundation of this model comprises various elements deemed essential to delivering superior patient care. These include the quality of nursing leadership and coordination and collaboration across specialties, as well as processes for measuring and improving the quality and delivery of care.

“To earn Magnet recognition once was a great accomplishment and an incredible source of pride for our nurses. Our repeated achievement of this credential underscores the foundation of excellence and values that drive our entire staff to strive harder each day to meet the health care needs of the people we serve,” said Becky Montesino-King, DNP, RN, NEA-BC, CENP, vice president and chief nursing officer, Baptist Hospital and Baptist Children’s Hospital. “Magnet recognition raised the bar for patient care and inspired every member of our team to achieve excellence every day.”

Magnet designation is widely accepted as the gold standard of nursing excellence. An organization seeking to reapply for Magnet recognition must provide documented evidence of how Magnet concepts, performance and quality were sustained and improved over the four-year period. Just 452 U.S. health care organizations out of over 6,300 U.S. hospitals have achieved Magnet recognition. Research demonstrates that Magnet recognition provides specific benefits to health care organizations and their communities. U.S. News & World Report’s annual showcase of “America’s Best Hospitals” includes Magnet recognition in its ranking criteria for quality of inpatient care. Health care organizations must reapply for Magnet recognition every four years based on adherence to Magnet concepts and demonstrated improvements in patient care and quality.
